For referral partners
We take referrals from physicians, hospitals, skilled nursing and rehab facilities, home health and hospice agencies, workers’ compensation case managers and health plans — by fax or phone, around the clock.
Two ways to send us a referral
Fax (800) 594-1226 — monitored around the clock; new faxes are pulled into our intake queue automatically.
Call (800) 304-7030 — we will take the order over the phone and send you the paperwork to sign.
Send the order and the supporting chart notes together. That single habit shortens turnaround more than anything else either of us can do.
Who we work with
Physicians & clinic staff
Order entry, e-signature, and a clear list of what each category needs so orders do not bounce.
Discharge planners & case managers
Equipment in place before the patient gets home — and walker and nebulizer closets for same-day discharge.
Home health & hospice
Reliable supply for wound, ostomy, catheter and enteral patients, and equipment your clinicians do not have to chase.
Facilities — SNF, ALF, rehab
Bulk referral handling, priority processing, asset tracking and scheduled pickups.
Workers’ compensation
24/7 order intake, carrier-specific billing, and audit-ready documentation for the adjuster's file.
Health plans & managed care
Clean electronic claims, documentation-first ordering, and member communication that reduces complaint volume.
What we are actually good at
- Depth in the recurring supply lines. 993 ostomy items, 787 compression, 517 wound care, 425 urological, 407 respiratory, 180 enteral. If your patient has been fitted to a specific product, we can usually supply that product rather than the nearest thing in the van.
- Documentation chased at our end. We know what each category needs and we call your office for the missing piece rather than letting the order sit.
- Insurance verified before delivery, with the patient told what their share will be — which is most of what your front desk currently fields calls about.
- 24/7 intake. Faxes arriving at 2am are in the queue at 2am.
- Same-day discharge equipment through walker and nebulizer closets on your unit.
- Patients told what is happening at each step, by text, email or phone.
What we need from you
Every category has its own documentation requirements and they are set by the payer, not by us. The pages above list them per specialty. The universal minimum:
- A written order signed and dated by the treating prescriber, with the item clearly identified (and the side, for anything bilateral).
- The diagnosis codes that support the item.
- Chart notes from a face-to-face encounter documenting why the patient needs it.
- Patient demographics and insurance information.
- Delivery address and a working phone number for the patient or caregiver.
If something is missing we will tell you exactly what and why — not “additional documentation required”.
Not sure where to start?
Call us and tell us what happened — a surgery, a new diagnosis, a discharge date, a supply you keep running out of. We will tell you what is usually needed, check your benefits, and ask your prescriber for the order if you do not have one yet.
