What each category needs

| CPAP / BiPAP | Sleep study, face-to-face evaluation documenting symptoms, and later the adherence data plus a follow-up visit showing benefit. |
| Hospital bed | Why an ordinary bed will not do — positioning need, head elevation requirement, traction, or frequent repositioning the patient cannot perform. |
| Support surface | Pressure injury risk or existing wound with stage, site and size; what lower-level surface was tried. |
| Surgical dressings | Wound location, type, number, measurements and change frequency. Quantities are derived from these. |
| Catheters | Type, French size, tip and daily quantity; justification for coudé, hydrophilic or closed systems. |
| Ostomy | The ostomy type and the specific product references the patient is fitted to. |
| Enteral nutrition | Documented impairment of the ability to take nutrition orally, the formula, the daily volume and the route. |
| Compression / lymphedema | Compression class in mmHg, garment style, limb, and the underlying diagnosis with measurements. |
| Wheelchair | Face-to-face mobility evaluation; why a cane or walker is insufficient; that it can be used in the home. |
| Bracing | Joint, side, diagnosis, and for post-operative bracing the procedure, date and range-of-motion settings. |
This table is a practical summary of common payer expectations, not a quotation from any specific coverage policy. Requirements differ between Medicare, Florida Medicaid and each commercial plan, and they change. We verify against the current policy for the patient in front of us every time.
