Discharge planners & case managers
Equipment waiting at home, or in the patient’s hands at the bedside — because a discharge that stalls for a walker is a bed you cannot use.
Two ways to send us a referral
Fax (800) 594-1226 — monitored around the clock; new faxes are pulled into our intake queue automatically.
Call (800) 304-7030 — we will take the order over the phone and send you the paperwork to sign.
Send the order and the supporting chart notes together. That single habit shortens turnaround more than anything else either of us can do.
Same-day discharge equipment
Our walker and nebulizer closets put stocked equipment on your unit. A patient is scanned in at the kiosk, leaves with the device, and the insurance intake runs afterwards at our end. Nobody waits for a delivery van to clear a bed.
Discharge-to-home equipment
For beds, mattresses, lifts, wheelchairs and commodes we schedule delivery against the discharge date so the equipment is set up before the patient arrives, not after.
- Tell us the discharge date as early as you have it — even provisionally.
- Tell us about access: stairs, lift size, doorway widths, and whether anyone will be home.
- Tell us who the caregiver is, because they are the person we need to train.
- Send the order and the chart notes together; we will call you if anything is short.
Bulk and repeat referrals
Send several at once by fax or portal. We de-duplicate and confirm each one back to you. Facility partners get priority processing and a direct route rather than the general queue.
What slows a discharge down, in our experience
- Prior authorization discovered late. Tell us the plan as soon as you have it and we will check whether the item needs authorization on day one rather than day three.
- Same-or-similar conflicts. A patient who already has equipment on file may be blocked from a similar item. We check this before ordering.
- Missing face-to-face documentation. Common on weekend discharges when the attending has gone.
- No working phone number. We cannot schedule delivery to a patient we cannot reach. A caregiver's mobile is worth more than a landline.

Call us and tell us what happened — a surgery, a new diagnosis, a discharge date, a supply you keep running out of. We will tell you what is usually needed, check your benefits, and ask your prescriber for the order if you do not have one yet.
